Attach anything that supports this

Receipts for a claim, a medical certificate for sick leave, a quote for something you need bought. Up to six files.

What happens next

You get a ticket number
By email, straight away. Quote it when you follow up.
It reaches the right desk
Leave and notice to management, claims and refunds to finance.
You are told either way
Approved or declined, in writing, with the reason. Do not book anything until leave is approved.

Before you send leave

Check the roster and say who is covering you.
Annual leave needs reasonable notice, and more than one person cannot be off the same shift.
Sick leave is still phoned in before your shift — this form does not replace that call.
More than two days off sick needs a medical certificate attached.

Before you claim

Attach the actual receipt, not a photo of the shelf price.
Get approval before spending anything over your limit — a claim is not approval after the fact.
Approved claims are paid with the next payment run.
